• Patrick McGoohan (Number Six) is the only actor to appear in all 17 episodes. In second place is Angelo Muscat (The Butler) who appeared in 15 of the 17 episodes. He did not appear in "Living in Harmony" and "The Girl Who Was Death".
• This episode was hurriedly produced when Patrick McGoohan was informed the show was being canceled instead of being renewed for a second season. Because of this, he brought Leo McKern back after 13 months following his appearance in "The Prisoner" (1967) {Once Upon a Time (#1.15)}. McKern had altered his appearance in the interim and was no longer as hirsute, so McGoohan devised a scene whereby the dead Number 2 would be resurrected by means of a process that required his face being shaved before final resuscitation could be achieved.
• Last show of the series.
• Patrick McGoohan (Number Six), Angelo Muscat (The Butler) and Peter Swanwick (The Supervisor) are the only actors to appear in both the first and last episodes of the series.
• "Fall Out" is the only episode in which the main filming location is named in the opening credits, as being Portmeirion, a hotel resort in north Wales near Penrhyndeudraeth. Its architect and proprietor Clough Williams-Ellis is also thanked in them.
