"Once upon a Time" references Shakespeare's Seven Ages of Man speech from "As You Like It", which begins "All the world's a stage". Number Two conducts psychological tests representing the seven ages, with the exception of the age of the lover to discover why Number Six resigned.
SPOILER: This is the only episode in which No 2 dies.
In the dialogue during the opening, which is the same as in "The Chimes of Big Ben", Leo McKern answers to Patrick McGoohan's query of "Who is number one" by emphasizing "*You* are, number six".
This was originally scheduled as the final episode of the first season. However, after the show was canceled, Patrick McGoohan used it as the springboard for the final episode he shot to sum up the story.
Contrary to all other uses of the distinctive "prisoner" font, there is a dot on the "i" of the word "Time" in the title card of this episode.
