Summary: Sam and Dean travel to an abandoned sanitarium where they find that the patients once revolted against cruel and unusual punishments given to them by the head doctor. The brothers search the area for lost college students, but the spirits make them go insane, and end up turning Sam against Dean.

MISTAKES/GOOFS

CONT: Dean's lighter went out of order once but some minutes later it was on again.
DATE: In a couple of scenes is shown a door with the internationally recognized biohazard symbol on it. The symbol was created in 1966 by Dow Chemical while the asylum is reported to have closed in 1964.

TRIVIA

This is the first time that iron is established as a spirit deterrent.
This episode takes place in a "small town" called Rockford, IL. Rockford is not actually a small town but is, in fact, the third largest city in Illinois with a population of 352,290.
Riverview Hospital, a large mental health facility used as the main set in the season 1 episode Asylum, has also substituted for a prison, a regular hospital, and an apartment building on different episodes of the show. This is evidenced by seeing the same distinctive staircase in different episodes.
Jared Padalecki and Jensen Ackles had Tom Welling hiding in the backseat of the Impala during shooting of some of the scenes.
