SPOILER: For the first two seasons of Enterprise, there are no fatalities among the crew. The first crew death was "Crewman Fuller" from a raid by Osaarian pirates in "Anomaly", the 2nd episode of Season 3.
According to Mike Sussman, an uncredited Brannon Braga actually wrote half of episode: #3.2, "Anomaly", particularly the airlock scene.
