Episode 15 - Goof (revealing mistake): When Hiro and Ando jump off the pickup truck, in the wide shot the actor playing Ando is obviously not James Kyson.
Episode 12 - Goof (continuity error): When Hiro meets up with Claire on the roof after completing his mission, make-up appears on Claire's face and vanishes again shortly after Arthur's arrival.
Episode 25 - Goof (revealing mistake): At the very end, when trailing season 5, someone is seen reading a paper with an article heading of "Fourth Mysterious Drowning ...". The text of this article is just several repeated paragraphs and bears no relation to the heading. The same text also appears in adjacent article
Episode 10 - Goof (factual errors): Throughout the episode people are shown looking directly at the eclipse as it progresses through various stages. In reality doing so can cause permanent eye damage.
