Summary: In the pilot episode of 'ER' overworked residents are the heroes of County General Hospital as they make life-and-death decisions on a daily basis. They include Dr. Mark Greene, a married chief resident who's being wooed by a lucrative practice; Dr. Douglas Ross, a charming, womanizing pediatrician who suspects a mother of abusing her young son; the down-to-earth Dr. Susan Lewis who must console a fragile cancer patient; Dr. Peter Benton, an intense surgeon who is forced into performing a delicate surgery he's not prepared for; and John Carter, an inexperienced medical student who bumbles his way through the cauldron of the emergency room. Julianna Margulies stars as Nurse Carol Hathaway, whose personal crisis takes the ER staff by surprise.

- Nearly fifteen years later, director Rod Holcomb would direct the show's final episode: "ER" (1994) {And in the End... (#15.22)}.
- The pilot wasn't shot in a set, but in an abandoned hospital (the Linda Vista Community Hospital in East Los Angeles). When the set was built, it was modeled after the real hospital, but it's not a exact replica: looking carefully it's possible to see several discrepancies. For example, the entire corridor that goes around the exam 3 room is absent in the pilot.
