CREW: At eleven minutes in, as Buffy shuffles down the hospital corridor, on the ceiling behind her you can see a lighting instrument with a black shroud around it to direct its light and mask it from the camera.

When the gang takes Buffy for a walk around the hospital grounds, she tells them she saw "Death". Xander advises her to refuse if Death offers to play chess with her. This refers to 'Max von Sydow''s epic chess match vs Death in Ingmar Bergman's Det sjunde inseglet (1957).
Joss Whedon designed Der Kindestod himself. He was an amalgam of Freddy Krueger, Nosferatu, and the boogeyman.
This was originally supposed to be a first season episode.
