Karen Davis is an American Nurse moves to Tokyo and encounter a supernatural spirit who is vengeful and often possesses its victims. A series of horrifying and mysterious deaths start to occur, with the spirit passing its curse onto each victim. Karen must now find away to break this spell, before she becomes its next victim.

Movie Goof (continuity error): The amount of liquid spilled from the ramen cup varies between shots.
Movie Goof (revealing mistake): After Karen sees the ghost image in the bus window, her head and reflection do not match.
SPOILER: There is a deleted scene after Toshio attacks Matthew, where Matthew becomes possessed by Takeo's ghost and kicks a visiting Susan out of the house. Takashi Shimizu cut it because it took focus off Kayako.
A sequel was greenlit on the Monday after the surprise success of the first weekend's box office of more than $39 million.
