After the brutal death of a friend, a group of friends find themselves in possession of a video-game called "Stay Alive," a blood-curdling true story of a 17th century noblewoman known as the Blood Countess. After playing the game when they know they shouldn't, however, the friends realize that once they die in the game - they die for real.

  • Movie Goof (factual errors): October finds a book called the "Malleus Demonium" and claims it's the Witches' Hammer. The ACTUAL Witches' Hammer is titled the "Malleus Maleficarum"
  • Movie Goof (continuity error): SPOILER: When Abigail, Hutch, October and Swink are outside on the balcony after Miller's death, once Swink removes his visor, it jumps from his hands to his head between shots.
  • Although the term "Blood Countess" is used in the movie's synopsis, it is never said within the movie. When Hutch and Abigail are hearing the story of Elizabeth Bathory from the author of the book "The Lady of Blood", the author says, "Thus began her obsession" (speaking of using the blood of the girls as a "Fountain of Youth", as Abigail says), to which Hutch replies, "The Blood Countess" (not in the theatrical version, but at approximately 1 hour 1 minute and 44 seconds on the uncut-version DVD).
  • Movie Goof (continuity error): After the scene in which Phineus takes a hit from a bong, during different takes the right shoulder strap on Abigail's top goes from up, to down, to back up.
