  • Movie Goof (plot holes): SPOILER: When Gwen is found dead under mysterious circumstances, Ted is allowed to perform her autopsy, even though he is the one who discovered her body and is therefore a possible murder suspect. In real life, Gwen's autopsy would have been performed by a pathologist who was not personally involved in the circumstances surrounding her death.
  • Movie Goof (revealing mistake): When the doctors slam the Asian man's corpse into the wall (on the gurney), it is clear that the actor puts his hands up defensively before he falls.
  • Due to disturbing behavior throughout - including violence, gruesome images, strong sexual content - the first theatrical trailer of the movie was digitally altered. It showed operation scenes without human organs, also Milo Ventimiglia's hands digitally erased in a love scene. But the later Red Band version was released without any alterations.See also Into the Blue and The Hangover.
  • Movie Goof (errors made by characters, possibly deliberate errors by the filmmakers): When Ted is performing Gwen's autopsy, the entire time the body block (under her neck) is left in place which would have made the autopsy difficult. Usually the pathologist will place it under the back, causing the arms and neck to fall backwards to stretch and push the chest forward to make it easier to cut open.
